The patient seeks treatment with full understanding that we do not process insurance paperwork and that all fees are the responsibility of the patient and due at time of visit.
Simplified Lab Work:
At Revita, we believe in straightforward communication and transparency. Lab work fees and structures can often appear complex, but we aim to ensure there are no miscommunications. It’s important that our lab work policy is clear and easily understood. The pricing below indicates the cost of labs if you choose not to bill your insurance for the services. While many patients prefer to bill their insurance directly for lab costs, please note that this process is separate and managed by LabCorp or Quest, independent of Revita.
Revita Lab Costs, If Billed Through Revita:
- Male Hormone Panels: $240
- Female Hormone Panels: $260
LabCorp or Quest Lab Billing:
Most insurance companies offer coverage for blood work, but some plans have limitations. If you opt to file blood work through your insurance and discover that they do not cover it, the bill becomes your responsibility. We want to ensure our patients understand this process. While we do not directly submit claims to insurance companies, both LabCorp and Quest are in-network and will handle the billing for your lab costs. At the time of blood collection, the phlebotomists will request your insurance documentation and manage the paperwork based on your insurance provider’s requirements.
We also provide the added convenience of having a Quest phlebotomist available in our office for those who prefer Quest Diagnostics. It’s important to note that if insurance denies coverage, the cost of the blood work billed via insurance could be $2,000 or more. Consequently, many patients choose to pay for the blood work through our office to avoid uncertainties about insurance coverage. We encourage all patients to decide whether they want to pay for blood work through our office before blood is drawn.
Any inquiries or concerns related to billing for blood work should be directed solely to Quest or LabCorp. It’s crucial for patients to determine which lab is preferred by their insurance company and offers the best coverage before their initial visit.
Notes Specific For hCG Bloodwork:
Please note that blood work for the hCG program is already included in hCG pricing and only assesses your basic blood counts, organ functioning, and thyroid functioning. The additional hormone panels are extensive and more expensive and those are the only panels billed directly to insurance.
